APICS Benefits Extension for Unemployed Members Form
Society headquarters will bill each member on his/her anniversary month. The billing will include both society and chapter dues. Payment is due within thirty days after billing. If, at the time of billing, the member is unemployed, and actively seeking work within the APICS business sector, his/her membership may be extended for one (only) six-month period, without cost. The member will apply for the extension through the local chapter, which must approve the request and waive the chapter dues assessment.

APICS members requesting an extension of benefits due to unemployment status should use this form. The completed form must be received at APICS Society within thirty days after expiration date. The form must be signed by the member and approved by the member's chapter president. (For complete rules visit www.apics.org.)

UNEMPLOYED MEMBER BENEFIT EXTENSION CRITERIA

1. UNEMPLOYMENT STATUS
    a. The member must be actively looking for work within the APICS business sector.
    b. May be a "name only" consultant, while looking for work.
    c. Retirees or buy-out participants are not eligible.

2. APPLICATION/VERIFICATION PROCESS
    a. The member will apply for the extension in writing, to his/her chapter president, and include Copy
        of the society's billing notice.
    b. Each chapter will determine its own method for verifying the member's status.
    c. Membership extension authority will be in the form of a letter to the society membership manager
        from the chapter president, or a designated chapter officer. It will include a statement of authority,
        the member's name and membership number, and be accompanied by a copy of the member's
        billing notice.

3. TIMING
    a. All statements of authority must reach society within thirty days after the member's expiration date.
